Pathogenesis of acne vulgaris

Development of lesions 
    • Small, hyperkeratotic plug of corenocytes in lower portion of follicular infindibulum  
    • Precursor for acne lesions 
    • Closed comedones (“whiteheads) 
    • Open comedones (“blackheads”) 
    • Inflammatory papules 
    • Pustules 
    • Nodules 
Factors leading to lesions 
  • Follicular hyperkeratinization 
  • Increased sebum production by sebaceous glands possibly due to increased androgens 
  • Increased amount of skin flora bacteria Cutibacterium acnes (C. acnes, formerly Propionibacterium acnes) 
  • Inflammation  
Risk factors for acne development 
  • Repetitive skin trauma 
  • High glycemic load diet 
  • Psychologic stress 
  • Insulin resistance
